Required Supporting Documents
For individuals aged 60 and over:
- Proof of age is required. Accepted documents include:
- Birth certificate
- Passport
- Driver’s license
- Alberta Health Card
For individuals with a permanent or short-term disability:
- A current medical note dated within the last 3 to 6 months is required. The note must confirm that the disability prevents the individual from driving.
- For short-term disabilities, the note should include an estimated timeframe for when the individual might be able to drive again.

- Download and complete the Community Access Program Application. Mail, drop off or email completed application forms to Town of Okotoks. (but this needs to be updated to the most current one that is fillable)
- Pick up an application form from the Okotoks Recreation Centre, Okotoks Municipal Centre or Okotoks Family Resource Centre. Mail, drop off or email completed application forms to Town of Okotoks.
Submit completed forms by:

Once your application has been reviewed and approved, an identification card will be mailed to you.
